The one decision that matters more than the design is what the code points at. Encode a short, permanent URL you control — ideally a page on your own domain that redirects — rather than a long tracking link or a document that will move. A printed QR code cannot be edited: once it is on a menu, a poster or ten thousand flyers, the destination is fixed for as long as that paper exists. A redirect on your own domain keeps the code alive when the target changes.
Size and placement decide whether it scans at all. As a rule of thumb the printed code should be at least a tenth of the scanning distance: about 3 cm for a code read at 30 cm, and closer to 20 cm for one on a wall read from two metres. Keep the quiet zone — the blank margin of roughly four modules on every side — clear of text and graphics, because scanners use it to find the code's edges.
Contrast matters more than colour. Dark code on a light background works; the reverse often fails, because most scanners expect dark modules. If you use brand colours, keep the code substantially darker than its background, and never place it over a photograph or a gradient.
Shorter data makes a denser code easier to scan. Every extra character adds modules, and a long URL crammed onto a business card becomes a fine grid that phones struggle with in poor light. Shorten the link first, then generate.
And always test before printing: scan the finished code with at least two different phones, one iOS and one Android, from the distance people will actually stand at, under the light they will actually have. A code that scans on your desk under a lamp can fail on a restaurant table at night.
